You might want to try and book it online before you go.
All of the major car rental places are represented at Heathrow. They are all going to offer you a good deal, poor quality crappy car rentals can’t afford to be at Heathrow, so I’d just shop based on features and price.
Insurance will be included, as it is a legal requirement to be insured while driving in the UK.
